Need a primary care provider?

Reminder: Patient registration opens Monday, March 5

Reminder the next wave of patient registration for the Algoma Nurse Practitioner-Led Clinic will open on Monday March 5, 2012 at 9 a.m.

The registration process is outlined below:

Who can register?

• Residents of all ages who live in Sault Ste. Marie and surrounding area
• Anyone who is not a registered patient of another physician or nurse practitioner in Ontario

How can you register?

• Electronic registration is encouraged and can be completed by visiting our website at www.algomanplc.ca
• Paper forms will also be available at the SSM Public Library Main Branch starting March 5 at 9 a.m. and will be available during library hours
• Individual forms must be completed for each person wanting to register

When can you register?

• Monday, March 5 at 9 a.m.
• Visit the Algoma NPLC’s website for updates on registration at www.algomanplc.ca

The Algoma Nurse Practitioner-Led Clinic is a new model of primary care delivery that brings together a multidisciplinary team of health care professionals and is led by nurse practitioners.

Patients of the clinic are registered to a nurse practitioner and have access to the clinic’s multidisciplinary team which includes a registered nurse, registered practical nurses, social worker, pharmacist, and administrative support staff.
