The Nurse Practitioner (NP) uses their enhanced clinical knowledge and skills to collaborate with the interdisciplinary team in providing comprehensive direct care to Long-Term Care (LTC) residents with specialized and complex needs. The attending NP works with residents, family/caregivers, and the health care team in the development, implementation, and evaluation of the resident’s plan of care. They provide leadership to our staff to enhance their knowledge, assessment skills, and ability to care for residents. They will lead and collaborate in research, education, and evidence-based practice initiatives to optimize resident, LTCH, and health system outcomes.

Qualifications

· Master of Nursing or equivalent would be an asset.

· Registration in good standing with the College of Nurses of Ontario, within the RN (Extended Class)

· 1 - 5 years of clinical nursing experience, experience in Long-term Care would be an asset

· Critical thinking with excellent communication, public speaking, and problem-solving skills and decision-making skills in assessment, diagnosis, and management of acute and chronic diseases.

· Knowledge of theoretical frameworks and research application

· Continuous learner who seeks out opportunities to maintain competency

· Experience with chronic disease management and geriatric care

· Ability to deal with crisis situation promptly and effectively

· Strong interpersonal, leadership and problem-solving skills

· Valid CPR and First Aid certification

· Knowledgeable in relevant legislation (LTC, OHS)

· Ability to integrate the home’s values into everyday practice

· Demonstrated effective team building skills

· Efficient time management and organizational skills

· Demonstrated ability to address the uniqueness of each patient and/or resident

· Experience with Microsoft Office including Excel, Power Point, Access, and Word.

· Experience working with electronic medical records
