Next wave of patient registration opens Tuesday, June 26

The next wave of patient registration for the Algoma Nurse Practitioner-Led Clinic will open on Tuesday June 26, 2012 at 9 a.m.

The registration process is outlined below:

Who can register?

Residents of all ages who live in Sault Ste. Marie and surrounding area.

Anyone who is not a registered patient of another physician or nurse practitioner in Ontario.

How can you register?

Electronic registration is encouraged and can be completed by visiting our website here

Paper forms will also be available at the SSM Public Library Main Branch starting June 26 at 9 a.m. and will be available during library hours
Individual forms must be completed for each person wanting to register.

When can you register?

Tuesday, June 26 at 9 a.m.

The Algoma Nurse Practitioner-Led Clinic is a new model of primary care delivery that brings together a multidisciplinary team of health care professionals and is led by nurse practitioners.

Patients of the clinic are registered to a nurse practitioner and have access to the clinic’s multidisciplinary team which includes a registered nurse, registered practical nurses, social worker, pharmacist, and administrative support staff.

“Our government has made unprecedented investments in health care in Sault Ste. Marie and we are continuing to deliver improvements to front-line care with the announcement that the Nurse Practitioner-Led Clinic is accepting another round of new patients that will be in addition to the approximately 1,000 they are already serving,” said David Orazietti, MPP. “Building on and protecting the tremendous progress we have made in increasing access to primary care is a top-priority and we will work tirelessly until every resident in Sault Ste. Marie has a doctor or nurse practitioner."
